Ready to connect with our team to learn about services and programs? Follow the steps below to book an intake assessment through our confidential virtual platform OnCall Health.  

How to Book an Intake Assessment 

  1. Click ‘Book an Intake Assessment’. Link will open a new window with booking page.
  2. Answer the preliminary questions.
  3. Select a service, provider and an intake assessment date and time.
  4. Create or login to your account using your email address.
  5. Complete your booking. A confirmation will be sent via email.

If you are having difficulty booking your appointment online, or need assistance, please contact our Client Support Team at (905) 450-1608 ext. 112. 

What to Expect at your Intake Assessment 

  • Your appointment will be with trained, qualified staff and/or supervised interns from Catholic Family Services Peel Dufferin.
  • Intake Assessment appointment are 50 minutes.
  • The purpose of the Intake Assessment is to assess needs and get to know you and your circumstances. Our staff will do their best to collaboratively determine next steps and develop a service plan.
  • You will have an opportunity to ask questions and share feedback during and after the appointment.
  • The services are free of charge and confidential. There are, however, circumstances when there are exceptions to confidentiality, which can be reviewed in our Welcome Document. Subsequent appointments (i.e. counselling and psychotherapy) may have a fee depending on your service plan, which will be shared and reviewed during the Intake Assessment appointment.

We ask you to review CFSPD’s Welcome Document prior to your Intake Assessment appointment. Our staff will be able to answer any questions you may have at that time.  

If you are unable to make your Intake Assessment appointment, we ask that you cancel 48 hours prior to the appointment start time so that appointment slots can be made available for other clients. Appointments can be cancelled and rebooked using your OnCall Health account or by contacting our Client Support Team at (905) 450-1608 ext. 112.
